Please collect copy of sales receipt, serial number photo, and bike photos prior to starting this form. You will not be able to proceed or complete the warranty claim process without uploading these items. If you do not have the sales receipt, or additional images, please upload a document file in the required upload field describing where you purchased the product with detailed reason for missing information.

All required fields, indicated by an asterisk (*), must be filled out in order for the warranty form to be submitted to our Litespeed Warranty/Repair Department! After your select the "Submit" button please be sure that you see a notification that your message has been sent. If you do not see the "Message Sent" notification, please check to make sure all required fields have been filled out - the required fields that have missing information or uploads will be indicated by a red outline.

Once your submission has been received you will receive an automated email confirmation to the email provided on this form. If you have not received an email confirmation within 15 minutes of your submission please check your warranty/repair form for any errors and re-submit.
